I have had problems with front tooth for 30 years. An initial root canal was not performed properly and I have had it redone and subsequent root surgery twice by expert endodontist and more recently successful peridontal pocket treatment. However the tooth is now causing problems again. It is throbbing and sensitive plus pain is spreading above gum line to roof of mouth. My dentist took x-ray two weeks ago and everything looked fine but this most recent flare up happened after a bit into something a bit hard a few days ago. Would extraction be the best course now?? — I don’t want to deal with another abcess. Perhaps the tooth has become cracked?? Also the tooth beside it seems to be acting up

I would say the only way forward while you have bone is to consider an immediate implant and see if you can save as much tissue as possible . You will have to shop around and i cannot recommend anyone in the States .This of coarse will be bone dependant .

If you have had endodontic surgery (apicectomy) the root of the tooth will have been shortened
If you have periodontal pockets then the attachment of the gum will have migrated up the root of the tooth
It is therefore possible that bacteria from the mouth will be able to penetrate between the tooth and gum or even above the tooth
This may not be visible on xray due to the bulk of bone that forms the palate
It is also possible that a fracture line in the root of the tooth could allow bacteria to track under the gum
Gum disease will often affect adjacent teeth as it is down to difficulty with cleaning which is rarely isolated to one tooth in the mouth
It may well be that this tooth has come to the end of the line but you will need to discuss the options for its replacement (denture, bridge or implant)if you do not want a gap
